Lead Licensed Practical Nurse (LPN) – Home Health

Meridian, Idaho

St. Luke’s Health System is seeking a Lead Licensed Practical Nurse (LPN) to join our Home Health team in Meridian. At St. Luke’s, we are committed to delivering high-quality, compassionate care while supporting our team members through collaboration, respect, and shared purpose.

Position Summary

Under the direction of a Registered Nurse or designated team leader, the Lead LPN provides skilled nursing care to patients in their homes. This role supports a manageable daily caseload and offers the opportunity to work independently while remaining closely connected to a supportive, interdisciplinary care team.

  • Average caseload of 4–5 patients per day

  • Occasional weekend coverage required

  • Schedule: 8:30 a.m. – 5:00 p.m.

Responsibilities

  • Provide direct nursing care to home health patients in accordance with established care plans

  • Serve as a clinical resource and support to the home health care team

  • Communicate clearly and effectively with patients, families, providers, and care team members

  • Collaborate with Registered Nurses and other disciplines to ensure coordinated, patient-centered care

  • Support team members as needed to ensure continuity and quality of care

Qualifications

  • Current Licensed Practical Nurse (LPN) license in the state of Idaho

  • Experience in home health, long-term care, or acute care preferred

  • Strong communication skills and a collaborative, team-oriented approach

  • Ability to work independently in a home-based setting
