What You Can Expect:

  • Full Time with Benefits

  • Provides day-to-day operations and oversight of post-acute liaisons, outreach coordinators, and patient management coordinators; covers vacancies or staffing shortages to ensure timely patient care and service continuity
  • Manages full cycle HR operations including hiring, staffing, performance management and employee coaching
  • Develops, reviews, and standardizes workflows, policies, and procedures for post-acute patient placement, referral management, and admission processes
  • Leads development of marketing and educational materials; conducts community outreach, rounding, and education to increase engagement through patient testimonials
  • Oversees data and analytics focused on admission conversions, challenges, referral metrics, and performance KPIs; drives service recovery and quality improvement
  • Assists with regulatory compliance, survey support, and standardized interfacility placement practices

 

Qualifications:

  •  Education: Bachelor's Degree or experience in lieu of degree
  • Experience: 4 years experience
  • License: Current & Valid Drivers License and Insurance and Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.


Post Acute Experience Preferred
