The Post Acute Liaison (Registered Nurse, Occupational or Physical Therapist) for Inpatient Rehabilitation plays a vital role in ensuring patients transition to the appropriate inpatient rehab facility at the right time. This position involves collaborating with other post-acute liaisons, processing referrals from acute care, and reviewing patient cases for clinical eligibility. The goal is to build trusting relationships with case managers and providers, and community partners to place patients, such as those with brain injuries, orthopedic conditions, or loss of function, in the optimal care setting for recovery, while growing our census through proactive outreach and positioning our inpatient rehabilitation services as the premier, trusted option for optimal recovery.


What You Can Expect:

  • Reviewing referrals for appropriate placement
  • Fostering the relationship between patients and the facilities providing their care
  • Meeting with patients to describe our hospital and how the services we provide will benefit them
  • Working closely with providers and case managers for possible referrals
  • Building and maintaining strong relationships with referring providers, case managers, discharge planners, and external vendors to generate referrals, grow census, and establish our inpatient rehab hospital as the trusted choice for post-acute recovery in the community
  • Schedule: Monday-Friday, some evening and weekend coverage

 

Qualifications: 

  • Education: 
    • ASN/ADN or higher, or degree from an accredited Occupational or Physical Therapy program
  • Experience: 
    • One (1) year relevant experience
  • Licenses: 
    • Current RN licensure from the State of Practice or valid license to practice physical therapy or occupational therapy in State of Practice 
    • Valid Driver's license and vehicle insurance
  • Certifications:
    • Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross
  • Preferred Qualifications:
    • Liaison experience
    • Post acute care experience
