As a Family Medicine RN, you'll work alongside an interdisciplinary care team to provide high-quality, patient-centered care while supporting patients with complex medical and social needs. Your work will combine direct patient care, care coordination, and population health initiatives to ensure patients receive the right care at the right time.

What You'll Do

Your role will include a balance of clinical care, care management, and quality improvement initiatives:

• 25% Direct Patient Care & Clinic Support
  • Provide hands-on nursing care and support daily clinic operations.
  • Assist providers with patient assessments, procedures, education, and follow-up care.
  • Triage patient concerns and coordinate timely interventions.
  • Collaborate with the care team to ensure efficient patient flow and exceptional patient experiences.
• 50% Care Coordination & Patient Navigation
  • Coordinate care for medically and socially complex patients across the Family Medicine practice.
  • Serve as an advocate and resource for underserved and vulnerable patient populations.
  • Connect patients with community resources and support services to address barriers to care.
  • Collaborate with physicians, behavioral health, social work, specialists, and community partners to improve continuity of care.
  • Promote preventive care, chronic disease management, and successful transitions between care settings.
• 25% Population Health & Quality Improvement
  • Support clinic quality initiatives and value-based care programs.
  • Monitor quality metrics and identify opportunities to improve patient outcomes.
  • Assist with panel management, preventive screenings, chronic disease outreach, and care gap closure.
  • Use data to support population health strategies and continuous quality improvement efforts.

What We're Looking For

Required Qualifications

• Education: Associates or Bachelor's of Nursing Degree 
• Experience: 1+ years’ experience
• Experience in family medicine, primary care, ambulatory care, community health, care management, or population health preferred.
• Strong assessment, critical thinking, and patient education skills.
• Excellent communication and collaboration abilities.
• Passion for serving diverse, underserved, and vulnerable patient populations.
• Licenses/Certifications: Current RN licensure from the State of Practice AND Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
