In this role, the Hospice LPN provides direct patient care within the scope of practical nursing under the direction and supervision of a Registered Nurse or licensed provider, collaborating with the interdisciplinary Hospice team to meet individualized patient needs.

Schedule

  • Friday, Saturday, Sunday
  • Two 10‑hour shifts and one 12‑hour shift
  • Rotating on‑call responsibilities

Core Responsibilities

  • Provide skilled nursing care to hospice patients in accordance with the established plan of care, under the direction of a Registered Nurse or licensed provider
  • Perform clinical interventions and treatments within LPN scope of practice, including medication administration as prescribed
  • Observe, document, and report patient condition, symptoms, and responses to care
  • Support comfort measures, activities of daily living, and symptom control
  • Communicate effectively with patients, families, and members of the interdisciplinary Hospice team
  • Participate in care coordination and contribute to the modification of care plans as directed


Qualifications

Education

  • Graduate of an accredited Licensed Practical Nurse program

Experience

  • Open to New Grads
  • Hospice, home health, long‑term care, or acute care experience helpful

Licenses & Certifications

  • Current Licensed Practical Nurse (LPN) license in the state of practice
  • Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross

Additional Requirements

  • Valid driver’s license with proof of auto insurance
  • Ability to travel to patient homes within the service area
