Telehealth Patient Specialist – Ambulatory Care & Women's Services
At St. Luke's, our team members are driven by compassion, innovation, and a commitment to delivering exceptional patient care. Join our Ambulatory Care as a Telehealth Patient Specialist, where you'll play a vital role in supporting patients through remote patient monitoring, care coordination, and telehealth services. In this position, you will help improve access to care, support patient engagement, and empower patients to better manage their health from home while partnering with multidisciplinary teams across multiple service lines.
Schedule
- Full-Time
- Three 12-hour shifts per week
- 7:00 AM – 7:30 PM
Rotating weekends and holidays required
Pay Range: $18.37 – $20.27 per hour, depending on experience
What You Can Expect
As a Telehealth Patient Specialist, you will serve as an important connection between patients and their care teams. In this role, you will:
- Onboard patients into remote patient monitoring, chronic care management, and Women's Services programs as applicable
- Monitor patient participation and assist with adherence to established care plans
- Support patients with the setup and use of remote monitoring equipment and technology
- Coordinate scheduling, follow-up appointments, and patient outreach activities
- Communicate effectively with patients, providers, nurses, and interdisciplinary care teams
- Utilize medical terminology and healthcare knowledge to support patient interactions and documentation
- Support initiatives focused on improving patient outcomes, enhancing patient experience, and reducing avoidable hospital utilization
- Provide compassionate support to patients across a variety of ambulatory and women's health care settings
Qualifications
Education
- High school diploma or equivalent required
Preferred Qualifications
- Experience as a Certified Nursing Assistant (CNA), Unit Clerk, Medical Assistant, Patient Access Specialist, or in a similar healthcare support role
- Familiarity with medical terminology
- Experience working in a healthcare, ambulatory, telehealth, call center, women's health, or patient-facing environment
- Strong customer service, communication, and computer skills
