Lead Licensed Practical Nurse (LPN) – Home Health
Meridian, Idaho
St. Luke’s Health System is seeking a Lead Licensed Practical Nurse (LPN) to join our Home Health team in Meridian. At St. Luke’s, we are committed to delivering high-quality, compassionate care while supporting our team members through collaboration, respect, and shared purpose.

Position Summary
Under the direction of a Registered Nurse or designated team leader, the Lead LPN provides skilled nursing care to patients in their homes. This role supports a manageable daily caseload and offers the opportunity to work independently while remaining closely connected to a supportive, interdisciplinary care team.
Average caseload of 4–5 patients per day
Occasional weekend coverage required
Schedule: 8:30 a.m. – 5:00 p.m.
Responsibilities
Provide direct nursing care to home health patients in accordance with established care plans
Serve as a clinical resource and support to the home health care team
Communicate clearly and effectively with patients, families, providers, and care team members
Collaborate with Registered Nurses and other disciplines to ensure coordinated, patient-centered care
Support team members as needed to ensure continuity and quality of care
Qualifications
Current Licensed Practical Nurse (LPN) license in the state of Idaho
Experience in home health, long-term care, or acute care preferred
Strong communication skills and a collaborative, team-oriented approach
Ability to work independently in a home-based setting
